Sick (fit) notes

Employees can take time off work if they are ill. You will need to give your employer proof if you’re ill for more than 7 days. https://www.gov.uk/taking-sick-leave

If you have been ill for 7 days or less you do not require a doctor’s note. Please complete the Statutory Sick Pay (SSP) Form : SC2-Form

If you’re ill just before or during your holiday, you can take it as sick leave instead.

If you have been ill for more than 7 days then please use the form below to request a medical certificate (sick/fit note). Please allow 5 days for the processing of this form. You may be called or asked to book an appointment if neccesary.
