What is Social Prescribing?
Sometimes the things affecting our health and wellbeing are not purely medical.
Social Prescribing connects patients with practical support, local services, activities, and community organisations that may help improve wellbeing, independence, and quality of life.
Our Social Prescribing team can support patients aged 18 and over with a wide range of social, emotional, and practical concerns.
How Can a Social Prescriber Help?
Social Prescribers may be able to support with:
- Loneliness or social isolation
- Befriending services and community activities
- Supporting following loss of independence (for example, after stopping driving)
- Financial worries or debt support
- Some housing or social care concerns
- Accessing charity or community services
- Dementia support and wellbeing activities
- Carer signposting and support services
- General wellbeing and lifestyle support
Dementia and Wellbeing Support
Our Social Prescribing team may also be able to support patients living with dementia by helping them access:
- Community activities
- Dementia-friendly groups
- Wellbeing support
- Local services and charities
- Social engagement opportunities
Who Can Refer?
Patients can be referred by:
- GPs
- Nurses
- Pharmacists
- Reception and administrative staff
- Other members of the practice team
Patients may also ask a member of staff if they feel Social Prescribing support could be helpful.
