Sick (fit) notes
If you are unwell and need time off work, the type of evidence you need depends on how long you have been off sick.
If you have been unwell for 7 days or less
You do not need a doctor’s note.
You can self-certify your illness using a Statutory Sick Pay (SSP) self-certification form: SC2-Form

If you have been unwell for more than 7 days
If you have been ill for more than 7 consecutive days (including weekends and bank holidays), you may been a medical certificate (fit note) from a clinician.

- Allow up to 5 working days for your request to be processed
- You may be contacted for further information
- In some cases, you may be asked to book an appointment before a fit note can be issued
Fit notes and annual leave
If you become unwell just before or during a period of annual leave, you may be able to take this time as sick leave instead. This is subject to your employer’s policies.

- Fit notes are only issued when clinically appropriate
- Requests submitted via this form are reviewed by the clinical team
