What is an Annual Diabetes Review
An annual diabetes review is a routine check to help monitor your condition and identify any potential problems early.
Without regular monitoring, diabetes can increase the risk of complications affecting the heart, kidneys, eyes, nerves and feet. Many of these risks can be reduced through routine checks, timely treatment changes and ongoing support.
The review is carried out in two stages, which may take place on different dates.
Stage 1: Health Checks and Blood Tests
You will be invited to attend the surgery for an appointment with our nursing team to complete the following:
- Foot check
- Blood pressure check
- Height, weight and BMI
- Blood tests related to your diabetes
These checks help us assess how well your diabetes is being managed and identify any concerns early.
Stage 2: Medication and Results Review
Once your blood results are available, they will be reviewed by our clinical pharmacy team.
During this stage:
- Your results will be assessed
- Your diabetes medications will be reviewed
- Any changes needed will be discussed with you
You may be contacted by phone or message if changes are recommended, or if further discussion is required.
Booking Your Review
We will contact you directly when your diabetes review is due.
You may be asked to complete an online questionnaire to help prepare for your appointment before booking.
If you:
- are unable to complete the questionnaire
- are unable to book using the link provided
- believe your diabetes review is overdue
please contact the surgery to discuss this with a member of the reception team.
